Adult & Teen Challenge offers a custom program that offers mental health, physical, spiritual and substance abuse services. Due to the client commitment needed to participate in a program of this length and our commitment to high quality of care please provide all of the following so that we can determine appropriate placement
- Medical Records that include
- HPI with diagnosis
- All labs
- List of current medications
- Current provider/nursing notes/case notes
- Additional Tests and Studies: To include but not limited to X-rays, CT, EKG, MRI
- An Arkansas ID or documentation necessary in order to obtain an Arkansas ID
